- Job Requirements
- Current Indiana nursing license
- Nursys verification within 30 days prior to start
- Licensed professionals must complete primary source verification within 30 days of start
- Pharmacology exam required prior to start, updated every 3 years
- Ascension Medical Surgical Exam required prior to start, updated every 3 years
- Ascension Core Competencies and EKG Exam completion required prior to start
- Health Statement required prior to first assignment, updated annually
- Fit Test required, preferably completed prior to start
- Documentation of Vaccination Series Completion for Hepatitis B, Varicella, and Influenza
- TB screening required prior to start and annually thereafter
- Ascension Skills Checklist to be completed within one year of start and updated annually
