Job Overview
The Wellness Program Coordinator is responsible for the overall implementation and success of various wellness programs focused on chronic disease prevention and management. Wellness Program Coordinator responsibilities include: completing Health Education referrals, addressing telephone calls for the Health Education line, patient screening and assessment processes, enrolling and scheduling program participants, tracking of patient data, tracking and monitoring patient’s progress, ensure high retention rates through patient engagement and follow up, and ensuring that data collection and reporting processes are accurate and valid. This role will ensure all wellness programs are following proper protocol adherence at all times.
The Program Coordinator will also provide care coordination of program participants including scheduling medical appointments, addressing patient care gaps, referral management, and linkages to community resources. Program Coordinator is responsible for relationship building with patients; specifically utilizing Motivational Interviewing skills to foster positive environment for participant.
Minimum Requirements:
1. High School diploma or GED required; Bachelor’s degree preferred.
2. English/Spanish required due to population served.
3. CPR/First Aid certificate preferred.
4. Proficiency in MS Office applications, including MS Work, MS Excel and Outlook required.
5. One-year of experience or internship preferred in health care field or working in a customer service setting or administrative, clerical and technical support services.
Compensation
$24.00 - $28.15 hourly
