Job Description
Coordinates the referral development and preāadmission patient assessment process.
Provides clinical assessment for potential clients including obtaining information from patients, families, physicians, therapists, discharge planners/care managers, and nurses that identify rehabilitation and/or skilled nursing needs and potentials. Through interactions with the interdisciplinary team achieves optimal clinical, satisfaction and resource outcomes.
Discusses K4 referrals with physicians and together determine appropriateness for the K4 program.
Ensures the completion of the entire "transition" process, including K4/TCU admission criteria compliance, insurance verification and precertification, discharge paperwork, and admission to K4/TCU. Communicates the discharge/admission process steps as appropriate to the acute and post acute unit disciplines.
Acts as the patient advocate for post acute care needs by providing patient/family education regarding K4/TCU admission criteria, patient/family interactions/updates regarding post acute plan of care, coordinating the discharge from acute care to K4/TCU, and following up with patients after admission to K4/TCU to assure a smooth transition.
Collects and analyzes outcome data. Establishes and monitors a referral source database and patient/customer satisfaction monitoring methods. Plans actions for improvement and makes recommendations to the appropriate team for additional outcome and process improvement.
Collaborates with other UHS, Inc. organizations and community health care agencies in planning, implementing, and evaluating postāacuteācare health care delivery services.
Maintains an active role in community education and internal education to referral sources to ensure an effective transfer process for patients referred and/or admitted to the unit.
Maintains an active role in the community and region as a representative of UHS Hospitals to market Sandiford Rehabilitation Center program, gain experience, and to enhance the image of the department and UHS Hospitals.
Understands and articulates reimbursement/funding strategies. Identifies and facilitates appropriate alternative levels of care or strategies to assure quality while decreasing costs.
Utilizes the computerābased case management/discharge planning system.
Education/Experience
Minimum Required:
A graduate of an accredited school of Nursing
Three (3) years of clinical experience
Preferred:
Previous experience in Medical Rehabilitation
Previous Marketing/Community Relations experience
License/CertificationMinimum Required:
Current New York State Registered Nurse License
CPR within 3 months of hire

Work Environment
- Work is performed in an office environment and patient care areas which require working in proximity to other employees and patients. Rooms are air conditioned and heated.
- Frequent sitting at a computer, walking, and standing.
- PPE (Eyewear, Masks, Face shields, Shoe covers, and Gloves) as required by UHS Hospitals Infection Control policies.
