Home Care RN Case Manager

Education/Experience

Minimum Required:

• Graduate of an approved RN program OR a non‐approved program supplemented by content equal to an approved program.
• Two years' experience as a Registered Professional Nurse required OR equivalent of education and experience.

Preferred:

• Bachelor's degree in Nursing.
• Med/Surgical/Certified home care experience.

License/Certification

Minimum Required:

• License and current registration to practice as a Registered Professional Nurse in New York State.
• CPR Certification required upon hire.

Work Environment

• Work performed in Patient Homes in a wide variety of unpredictable, non‐sterile environments (furniture, other residents, icy walkways, pets etc.).
• PPE: Gowns, masks, bags, gloves available as needed and per policy.
• Requires ability to: Lift and/or carry a nursing bag and plastic bin each weighing up to 15 lbs. in addition to a laptop weighing 4 lbs.; Walk up flights of stairs carrying nursing bag/supplies and laptop; Stand for 30 minutes or longer at one time while working; Lift, push, pull a minimum of 50 lbs. without restriction; Independently transfer or assist patients to move, stand or walk in a safe manner.

• Clinical ‐ Access to patient information that is not restricted (an example of restricted information is ARU) ‐ Level 3.
