MDS Assessor

A skilled nursing facility in Woodside, Queens that specializes in short-term, post-acute, and long-term care is looking for a full-time MDS Assessor to join our facility! REQUIREMENTS: • MUST HAVE PRIOR EXPERIENCE • MUST HAVE Valid NY Registered Nurse RN License • Long-term care experience required • Must be highly organized, professional & motivated • Should have solid computer skills • Managed Care experience DUTIES: • Completing accurate MDS and assessments • Collaborates with the Interdisciplinary Team to assess the needs of the residents. • Monitors MDS and care planning documentation for all residents; ensures documentation is present in the medical record to support MDS coding. • Monitoring & auditing clinical records, ensuring accuracy & timeliness. • Informing the Director of persistent issues related to non-compliant documentation. • Protecting the confidentiality of Resident & Facility information at all times.
