Job Overview We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for our patients. This position requires strong clinical skills, attention to detail, and a commitment to patient care. The Pharmacist will be responsible for providing medication therapy management, counseling patients on medication use, and collaborating with healthcare professionals to optimize patient outcomes. Responsibilities • Review and interpret prescriptions to ensure accuracy and appropriateness of medications. • Provide immunizations and manage IV infusion therapies as needed. • Conduct acute care assessments and collaborate with healthcare providers on patient medication plans. • Manage inventory effectively, ensuring that medications are stocked and stored according to regulations. • Utilize pharmacy software for prescription processing, record keeping, and inventory management. • Educate patients on medication administration, potential side effects, and proper usage. • Compounding medications as required, adhering to safety standards and protocols. • Maintain up-to-date knowledge of medical terminology and physiology to support patient care. Experience • A Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy school is required. • Active pharmacist license in the state of practice. • Experience in acute care settings is preferred but not mandatory. • Familiarity with pharmacy software systems is essential. • Knowledge of compounding medications and inventory management practices is advantageous. • Strong understanding of medical terminology and physiology is beneficial for effective patient communication.
