Wage Range: $74.87 - $97.33 per hour
Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.
Job Summary:
Responsible for monitoring drug therapy for outpatients. Provides pharmaceutical consultation to physicians, other health care workers, patients, and the general public. Responsibilities also include the preparation and dispensing of pharmaceuticals in the pharmacy.
Primary Duties:
• Performs a review of each medication order assuring safe and effective dosage, route, and rate/frequency of administration. Also ensures minimization of preventable drug interactions, therapeutic duplication, allergic reactions, and adverse drug reactions.
• Performs P&T established therapeutic interchanges.
• Provides medication teaching/counseling to patients as required.
• Understands Pharmacoeconomics and actively promotes cost effective drug therapy.
• Prescribes and monitors pharmacotherapy within the parameters of the applicable CDTA or consultation request. This includes integrating provider services within the patient’s broader healthcare treatment plan. Documents all care provided and interventions prescribed or provided in the medical record. Communicates essential information as necessary to other providers.
• Reviews and approves medication refill requests under the applicable protocols on behalf of defined EHMG provider groups.
• Consults on medication renewal tools and workflows as identified by pharmacy and clinic leadership. Provides drug information to health care providers in reply to specific requests in a timely fashion and presents the information in a form that aids the decision-making process for a specific patient where applicable.
• Performs other duties as assigned.
REQUIREDfor the position:
• Current Washington state pharmacist license
• Bachelor’s degree
• Math skills necessary to perform drug dosing and compounding calculations
• Current Healthcare Provider BLS – American Heart Association by start date
DESIRED:
• Resident training, Doctor of Pharmacy degree or equivalent experience
• Preceptor license
