Registered Nurse Assessment Coordinator (RNAC)
Full-Time | Fully Remote – Supporting Multiple Facilities in Pennsylvania

We're seeking a skilledRegistered Nurse Assessment Coordinator (RNAC)to support multiple long-term care facilities across Pennsylvania in delivering the highest standard of care — all from the flexibility of a fully remote role.

Key Responsibilities:
•
Accurately coordinate and manage timely completion of MDS assessments, RAPs, and Triggers across assigned facilities
•
Develop, update, and oversee care plans and MDS-related policies
•
Ensure regulatory compliance and participate in survey readiness activities
•
Use clinical judgment and data to assess resident care needs
•
Collaborate remotely with facility DONs, interdisciplinary teams, and family members
•
Provide support and guidance to on-site staff, ensuring proper documentation and care coordination
•
Deliver remote training and mentorship to facility staff on assessment practices
•
Monitor for accuracy, regulatory compliance, and policy adherence
Qualifications:
•
Active RN license in Pennsylvania (required)
•
Minimum 2 years of RNAC experience in long-term care or skilled nursing
•
Deep knowledge of MDS, PDPM, managed care, and bundled payment systems
•
Strong communication, leadership, and computer skills
•
Familiarity with regulatory requirements and the survey process
•
Ability to lead, educate, and support interdisciplinary teams remotely
