Responsibilities: • Conduct comprehensive assessments of patients' health status and develop individualized care plans. • Administer medications, perform treatments, and provide wound care as prescribed by physicians. • Educate patients and their families on disease management, treatment plans, and self-care techniques. • Monitor patients' progress and adjust care plans as needed to promote optimal health outcomes. • Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and holistic care. • Document patient care accurately and maintain confidentiality of patient information. • Provide emotional support and counseling to patients and their families to promote well-being and coping strategies. • Participate in interdisciplinary team meetings and contribute to the development of quality improvement initiatives.
