Title: Social Services Designee
Job Duties: The Social Services Designee is responsible for the ongoing assessment of each resident’s social service needs. Will assist with the development and implementation of a plan of care to meet identified needs. Responsible for providing medically related Social Services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident. The Social Service Designee will function as a participating member of the IDT (Interdisciplinary Team).

QUALIFICATIONS:
- Must obtain a BA Degree in Social Work, Psychology, or a related field.
- Social Services Designee Certificate Required.
- Three years experience in Mental Health or Related Fields.
- Demonstrated ability to assess client needs, implement treatment strategies, and provide individual, group, and family counseling and education.
- Displays competent verbal and written communication skills. Have effective problem-solving skills, with knowledge in Microsoft Word, Excel, and PowerPoint Presentations. And the ability to work cooperatively with facility staff and the public.

The salary range listed below represents the minimum and maximum base pay per hour at the time of posting. Final salary offered to the candidate selected for the position will be based on factors including but not limited to candidate's skills, experience, licensure, and program acuity.Â
Pay Range:
$28—$30 USD
