UPMC Central PA is seeking full-time adult and senior care physicians to join our growing post-acute
care team in Lancaster County and play an integral role in helping to provide high-quality,
compassionate care to our geriatric population. UPMC Central PA Post-Acute team provides care in
several skilled nursing facilities across the county. About the Position: Provides direct patient
care in multiple facilities across the aging continuum to include skilled nursing, rehab, personal
care/assisted living, independent living and clinicInteracts with facility leadership, hospital
discharge team, care management and other specialtiesCollaborates with and supervises Advanced
Practice ProvidersEPIC EMR, online diagnostics including PACS imaging and laboratory resultsOn-call
rotation, all phone coverage, shared equally with partners. Monday – Friday (No
weekends)Â Professional Skills:Â Board certified/Board eligibleTeam orientated with the ability to
provide personalized patient care and demonstrate clinical outcomesCommitment to providing
