Summary About the Position: This position is located in Honolulu, HI at Tripler Army Medical Center in the Directorate of Behavioral Health. Tripler Army Medical Center is a tobacco-free workplace. Duties Conducts expert clinical assessments and interviews to identify high-risk diagnoses and develop complex treatment plans. Develop and implement individualized treatment plans, monitor medication effects, and provide psychoeducation, skills training, and therapeutic interventions. Facilitate care transitions and collaborate with internal and external providers to ensure continuity and quality of care. Lead weekly treatment planning meetings, analyze patient data to identify trends, and recommend program improvements. Synchronizes care between DHA command and multidisciplinary teams to ensure seamless patient transitions. Chairs weekly At-Risk/Multi-Disciplinary Treatment planning meetings to align medical and Substance Use Disorder clinical care. Analyzes patient data and metrics to identify trends, improve program efficiency and update clinical databases. Monitors medical appointments and support services to ensure cost-effective, continuous care across multiple systems. Requirements Conditions of employment Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation. This position may be designated "Mission Essential." In the event of severe weather conditions or other such emergency type situations the incumbent is required to report to work or remain at work as scheduled to support mission operations. This position has a mandatory seasonal influenza vaccination requirement and is subject to annual vaccination. Employees assigned to a medical treatment facility are required to be screened for applicable immunizations or pre-existing immunity based on occupational risk, regardless of job series. Hepatitis B immunization is required for all positions with direct patient contact. Qualifications Who May Apply: US Citizens In order to qualify, you must meet the education and experience requirements described below. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). You will receive credit for all qualifying experience, including volunteer experience. Your resume must clearly describe your relevant experience; if qualifying based on education, your transcripts will be required as part of your application. Basic Requirement for Behavioral Health Case Management: Degree: Bachelor's or graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position. This degree must be from an educational program accredited by an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below: Specialized Experience: One year of specialized experience which includes conducting behavioral health assessments/interviews; coordinating behavioral health care plans with a multidisciplinary team; and evaluating appropriateness, quality, or cost effectiveness of care. This definition of specialized experience is typical of work performed at the second lower grade/level position in the federal service (GS-11). Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted.
