**Senior Care Physician - Geriatrics opening in WI** **Located in Appleton, 33m to Green Bay** **Full time and Part time positions available** **Accepts H1 Visa** Job Description: Our Medical Group, Wisconsin's largest non-profit healthcare system is seeking a Senior Care Physician to join our established team. You will be responsible for working with PALTC patients in community living facilities developing relationships while administering high quality patient care. Position Details: Full-time or Part-time options available with flexible schedules! Locations: 1 Physician needed for the Fox Valley and 1 Physician needed for Berlin/Oshkosh The team consists of 5 Physicians and 8 APC's On-Call by phone only Patient census: 8-12 Support staff includes RNs and schedulers who help to coordinate provider visits, communicate with facilities, and triage phone calls. Senior Care Services: Visits completed at patients location (Nursing Home or Assisted Living) Acute visits, compliance visits, & discharge visits for short and long-term residents Advance Care Planning & Goals of Care discussions Minor procedures at bedside Community Overview: The Fox Cities of Wisconsin includes 19 communities along the Fox River in East Central Wisconsin, including Appleton, Neenah, Menasha, Kimberly and a number of additional villages and townships.
