The Role: Case Management Program Manager

Location: Remote 


About This Role:

As the Case Management Program Manager, you will be part of the Clinical Operations team.  You will oversee the day to day operations of our growing Case Management team, ensuring high quality and timely delivery of case management services. You will work in collaboration with both clinical and business leaders across the organization. In addition to owning daily operations, you will play a key role in strategically shaping case management processes and tools as we evolve and grow.


What You’ll Do:

  • Plan, lead, and manage our team of Case Managers to meet Hazel’s contractual requirements and maintain a high standard of quality
  • Directly oversee our Case Management Supervisors and serve as the go-to for escalations and business decisions related to case management, including processing referrals, identifying community resources, hiring needs, and more
  • Participate in cross-collaborative conversations with our Clinical, Customer Success, Growth, and Product teams to serve as the ‘voice’ of case management as we grow our business
  • Outline new processes, define escalation protocols, and train team accordingly
  • Accountable for efficiency and quality metrics across the team
  • Additional duties as assigned

Your Background:

  • 5+ years of case management, including 3+ years in a leadership role
  • Clinical licensure required (LCSW or equivalent)
  • Must be comfortable in a changing, fast-paced, start-up environment 
  • Experience managing large, multi-state teams
  • Self starter and proactive problem solver
  • Tech savvy and comfortable using data to guide decision making
  • Experience working in K-12 education setting a plus
