ERP is an Equal Opportunity Employer - Disability and Veteran.Responsibilities: Work Schedule: Sunday to Saturday, 8, 10, or 12 hour shifts, days or nights, dependent upon the needs of the Pharmacy Job Specific Details and Tasks:The duties include, but are not limited to the following: • Responding to Cardiac Arrest and ‘Code Blue’ life support situations in an urgent manner. • Understanding and enforcing Joint Commission Medication Management standards. • Identifying and monitoring appropriate clinical and laboratory endpoints to determine the effectiveness and safety of prescribed medications. • Conducting and coordinating clinical investigations and research approved by a local or regional Investigational Review Board (IRB) and participating in outcome studies generated by the department of pharmacy and approved by the P&T Committee. • Ensuring prescription directions for use, dose, medication interactions, therapeutic duplications, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients (intervention with providers shall be made for noted discrepancy of above listed items). • Assessing metabolic needs, ordering, and preparing therapeutic enteral or parenteral nutrition products in the inpatient setting. • Conducting patient discharge interviews with appropriate medication counseling including storage, side effects, dose, administration, indication, drug-drug and drug-food interactions. • Maintaining controlled substance prescription requirements and dispensing procedures for inpatient orders, discharge prescription orders, and after-hour prescriptions. • Maintaining all documentation for controlled substance inventory, usage, returns and wastage on appropriate AF forms. • Properly signing out controlled substances for PCA pumps or controlled substance IV infusions. • Ensuring accuracy of all pharmaceutical products prepared or processed by technical staff including chemotherapy agents. • Attending medical rounds and intervening on pharmaceutical issues with medical staff. • Successfully completing the training competency checklist within 30 days of in-processing. • Participating in pharmacy student education including preceptorship. • Obtaining and maintaining privileged provider status IAW Credential Office requirements. • Completing peer reviews. • Ensuring that work areas are clean and safe according to applicable regulations. • Documenting significant pharmacy interventions and pharmacy interactions with prescribers. • Monitoring and managing pharmacotherapy requiring periodic adjustment due to pharmacokinetic characteristics (e.g., aminoglycosides, phenytoin, antithrombotic, vancomycin, warfarin). • Working closely with clinical staff in developing patient wellness programs to include but not limited to warfarin education and antibiotic stewardship programs. • Compounding medications as required using sterile techniques. • Attending and participating in meetings during normal duty hours, professional staff conferences and other appropriate professional activities such as, but not limited to the following: quality improvement meetings, professional staff meetings, commander's staff meetings, pharmacy flight meetings, and other meetings required by applicable regulations, MTF guidance, or as directed by the pharmacy flight commander or his/her designated representative and enforcing Joint Commission Medication Management standardsQualifications: Minimum Qualifications: • Degree/Education:Doctoral degree in Pharmacy from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE). Current employees may qualify for the position with either a Bachelor’s of Science in Pharmacy or a doctoral degree in Pharmacy from a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE). • Certifications in addition to Basic Life Support:HCW shall hold and maintain Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS). Certifications must be through American Heart Association or American Red Cross guidelines. Members must present proof of current certification(s). Computer-based training does not constitute fulfilling the training requirements. Must maintain currency at NO cost to the government • Experience:Must have minimum of one year experience as an inpatient pharmacist. Experience working in a military treatment facility preferred. • Board Certification:Board certification in one or more of the pharmacy specialties recognized by the Board of Pharmaceutical Specialties, or completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy, or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy. • Licensure/Registration:A current, valid, unrestricted license to practice Pharmacy in any one of the fifty States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.
