COMPANY GAS CARD Completes admissions and coordination of care for home health patients. We are seeking a dedicated Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills as well as the ability to function independently. You will be responsible for completing detailed initial evaluations, assessments or skilled nursing visits on hospice patients. Services could be provide in either a facility or /home environment to and you will need to determine specific nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change. MINIMUM QUALIFICATIONS: • Must possess current Illinois RN licensure • Possess a minimum of one years experience with (1) year experience • Current CPR • Valid IL driver’s license and auto insurance policy • HCHB EMR experience a plus • Must possess excellent clinical nursing and customer service skills • Fulfillment of criminal background check per agency policy • Pass pre-employment physical Benefits: • 401(k) • Dental insurance • Health insurance • Life insurance • Paid time off • Vision insurance
