We are excited to offer the opportunity to work as a Public Health Nurse in our Family Health Services department. We have current availability at our Southeast Public Health Center, located in Westmont, IL on a full-time basis. The hours are generally Monday through Friday from 8:00 am until 4:30 pm, with one to two days per week working until 7:00 pm.
β’ Provide WIC nutrition counseling and breastfeeding support
β’ Conduct comprehensive nursing assessments for pregnant/postpartum individuals and their infants
β’ Connect clients to medical, social, and community resources through referrals
β’ Deliver nursing services in clinic, community, and home settings
β’ Counsel individuals and families on health needs and concerns
β’ Educate clients individually and in small groups
β’ Develop individualized care plans
β’ Administer vaccines and apply fluoride varnish for infants and children
β’ Perform screenings (perinatal depression, social determinants of health, ages and stages)
β’ Complete documentation in the electronic medical record
β’ Participate in emergency response activities as assigned
β’ Registered Nurse (RN) licensed in the State of Illinois
β’ Bachelorβs degree in nursing preferred (Associate degree considered)
β’ One year of nursing experience preferred, not required
