This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Field Case Manager based in Canada.

Accountabilities:

The Field Case Manager will serve as a key point of contact for patients, healthcare providers, and program partners, ensuring coordinated and personalized support throughout the patient journey. Main responsibilities include:

  • Act as the primary contact for physicians, healthcare teams, and program stakeholders within the assigned territory.
  • Build and maintain trusted professional relationships with healthcare providers and clinic staff.
  • Educate healthcare professionals on available program services, patient resources, enrollment processes, and therapy support options.
  • Support patients by helping navigate treatment access, reimbursement processes, and administrative requirements.
  • Review patient documentation and ensure completion of required forms, authorizations, renewals, and records.
  • Manage special authorization processes by preparing accurate submissions and providing reimbursement guidance.
  • Provide regular updates and recommendations to healthcare providers regarding patient status, program changes, and best practices.
  • Adapt support approaches based on patient and provider needs while identifying opportunities for service improvement.
  • Collaborate with internal teams to ensure effective program delivery and high-quality patient support.
  • Participate in healthcare education activities and maintain compliance with program standards and procedures.
  • Requirements:

    The ideal candidate is a healthcare professional with strong interpersonal skills, attention to detail, and the ability to manage complex patient support processes independently. Required qualifications and skills include:

    • Registered Nurse (RN), Bachelor of Science in Pharmacy, science-related degree, or equivalent transferable healthcare experience.
    • Experience in case management, patient support programs, healthcare services, or related fields is an asset.
    • Knowledge of public and private third-party reimbursement processes.
    • Strong understanding of medical terminology and healthcare workflows.
    • Excellent communication skills in English; bilingual English/French communication skills are required for certain provinces.
    • Exceptional relationship-building and customer service abilities.
    • Ability to communicate confidently with physicians and healthcare professionals as a knowledgeable partner.
    • Strong organizational skills with the ability to manage multiple priorities and deadlines.
    • Ability to work independently in a remote environment while collaborating effectively with a broader team.
    • Strong analytical skills and attention to detail.
    • Proficiency with Microsoft Office and general computer applications.
    • Ability to adapt to changing priorities and manage sensitive or challenging situations.
    • Valid, unrestricted driver’s license and access to a reliable vehicle.
    • Ability to maintain a private, dedicated home office space suitable for remote work equipment.
