Home Health Care, RN Case Manager

Home Health Registered Nurse (Case Manager) Elgin, Hinsdale and Greater Chicago. The ideal candidates for these positions would possess excellent communication and time management skills, and have the ability to work both independently and as a member of a team. Candidates must possess strong clinical knowledge in various geriatric disease processes. RN Case Manageradministers skilled nursing care to patients requiring intermittent professional nursing services. Teaches and supervises the family, and other members of the nursing team. Each staff nurse performs nursing services for residents within a specific area. These services are performed in accordance with the physician's orders and under the direction and supervision of the Director of Clinical Services. Assume the responsibility for coordination of care. Monitoring through observations; patient, staff and physician feedback; written and verbal communication; quality improvement/quality assurance activities; records management. Responsible for Home Health Aide who may be working on a particular home care case. Responsible for communicating with all disciplines involved in the patient's care (i.e.: Physical Therapy, Occupational Therapy, and Speech Therapy). Minimum of 2 years home healthcare nursing experience. RN License in the State of Illinois Current Drivers License Current Auto Insurance Current CPR card.
