Partial list of job responsibilities: • Conduct scheduled, in-person meetings with clients in their homes • Organize medications and assess ongoing medical needs • Develop medication regimen and checklist • Dispense correct dosage of each medication at the correct time • Fill pill boxes or medication dispensers, update with new medication orders • Reconcile medications and notify clients when refills are needed • Coordinating with healthcare providers regarding changes in medication Reorder medications as needed and procure refills • Check labels, expiration dates, and warn patients or caregivers about potential interactions • Observe and document ongoing changes of condition (remotely viewable by family members) Private duty nurses serve as a liaison between the client, family members, and healthcare providers to ensure seamless communication and the best possible care plan for the client.
