Registered Nurse - RN - Home Health Care Field RN

About the Position • RN Field Case Managers are responsible for comprehensive assessments in the patient’s home, including the completion of OASIS using electronic documentation. • In addition to OASIS assessments, RN Field Case Managers perform skilled nursing visits as needed and are responsible for delegation and coordination of care with LVN and HHA team members • Must be fully skilled in lab draws, catheter placement, wound care , wound vac and IV Requirements • Current Texas Registered Nurse license • Current Texas driver's license • Automobile liability insurance issued in Texas • Minimum 2 Years of Home Health nursing experience.
