Job Title:Staff Pharmacist Report to: Pharmacist in Charge SUMMARY Currently seeking a staff pahrmacist to assist in our Dallas facility who may be willing to travel when needed to other company facilities. ESSENTIAL DUTIES AND RESPONSIBILITIESinclude the following. Other duties may be assigned. · Receiving new prescription orders orally from a prescriber or other person authorized by law. · The interpretation of clinical data. · Final inspection of all packaged drugs prior to dispensing to the patient Review and verification of all controlled medications received from drug wholesalers. · Accurate and timely completion of all required documents, forms, and reports as outlined in the company policy and procedure manual. · Maintain the prescription department as clean, safe, and secure. Prescription Verification 1 · Review and evaluation of all prescriptions and the assurance of accuracy in the patient’s prescription record. · Perform Drug Utilization Review of all prescriptions received · Consultation with patients or their agents in regard to drug or medical information. · Consultation with any prescriber or their authorized agent. · Transfer prescriptions to and from other pharmacies or institutions · The pharmacist shall directly oversee and supervise the activities of all pharmacy interns and technicians. Prescription Verification 2 · Review and evaluation of all prescriptions and the assurance of accuracy in the patient’s prescription record. · Ensure Drug Utilization Review of all prescriptions has been completed prior to dispensing/shipping · Ensure the accuracy and completeness of prescription orders that have been filled · Confirm calculations for all compounds · Maintains control of the controlled substance storage and logs controlled medications being added or removed from storage · The pharmacist shall directly oversee and supervise the activities of all pharmacy interns and technicians including (but not limited to) the loading of the automated dispensing machine, filling of prescription orders and confirming the accuracy of drug selection. Non Sterile Lab Pharmacist · Perform and or confirm pharmaceutical calculations · Perform quality assurance for completed compounds · Maintains control of the controlled substance storage and logs controlled medications being added or removed from storage · Create new or amend existing compound formulations · Verify all new chemicals received for accuracy, potency and quality · Review and release sterility tested compounds from quarantine Sterile Lab Pharmacist · Oversee, assist and compound sterile preparations · Verification of compounding medication accuracy based on ingredients, weights and finished product · Perform environmental monitoring of clean room (finger tip plating and surface plating) · Perform visual inspection of finished sterile drug preparations · Ensure that compounded medications have been properly compounded, packaged, labelled * Ensure proper storage of compounded medications · Oversee the cleaning and maintenance of equipment and work areas · Maintains control of the controlled substance storage and logs controlled medications being added or removed from storage · Ensure compliance with all company procedures, federal and state laws, rules and regulations related to compounding sterile products · Ensure department staff follows all safety procedures and guidelines for preparation of sterile compounds EDUCATION and/or EXPERIENCE Graduate of an accredited College of Pharmacy. Clear and active pharmacy license issued by the State of Texas Board of Pharmacy.
